When Should You Contact Us?
Leaving the Australian Defence Force (ADF) is a big step, and your health and wellbeing are a top priority during this time. That’s where Transition Medicals come in. This process is designed to make sure you’re supported both before and after you leave the ADF, so there’s no disruption to your healthcare. There are two main parts to the journey—one before you discharge, and one after.
Transitioning to civilian life is a big change, and your health shouldn’t be something you have to worry about. Transition Medicals are here to make sure you’re looked after every step of the way—before you leave, and after you’ve moved on.
Before You Leave the ADF (around 6 months out)
This is all about getting you set up for a smooth handover. You might be referred by your ADF Medical Officer through BUPA, or you can refer yourself. During this consultation, a healthcare professional will sit down with you to understand your current medical and allied health needs.
They’ll help make sure any treatments or surgeries you need are completed before you leave the service. You’ll also get advice on anything that still needs attention and how to manage it. Importantly, they’ll walk you through what medical support you’re entitled to after discharge—things like how to access the Provisional Access to Medical Treatment (PAMT) program, allied health services, household support, and incapacity certificates.
They can even help you lodge provisional claims early, so you’re not starting from scratch once you’re out.
After You Discharge (around 1 month later)
Once you’ve officially left the ADF, the focus shifts to putting your care plan into action. Since ADF-issued referrals and prescriptions don’t carry over, this consultation helps bridge that gap.
You’ll get new referrals and short-term prescriptions to keep things moving. If you’re eligible, PAMT will be activated, and where possible, you’ll be connected with a local GP to continue your care. The goal here is simple: no gaps in your medical support.
